Excessive foreskin in men may increase the risk of urinary system infection, affect the quality of sexual life, induce local inflammation or tumors, and in severe cases may lead to phimosis incarceration. Prepuce is mainly related to congenital developmental abnormalities, repeated infections, poor hygiene habits and other factors. Typical manifestations include smegma accumulation, difficulty urinating, and erection pain.
1. Urinary system infection
Excessive foreskin can easily cause smegma accumulation, which can breed bacteria and cause balanitis or urethritis. Long-term chronic inflammation may cause retrograde infection to the prostate and bladder, causing symptoms such as frequent urination and urgency. The foreskin needs to be washed every day. In case of acute infection, cefixime granules, levofloxacin tablets, amoxicillin-clavulanate potassium dispersible tablets and other drugs can be used as directed by the doctor.
2. Sexual dysfunction
An excessively long foreskin may reduce the sensitivity of the glans, leading to weak erections or premature ejaculation. Some patients suffer from sexual intercourse pain due to foreskin incarceration. It is recommended to improve through circumcision, use compound polymyxin B ointment to prevent infection after surgery, and use dapoxetine hydrochloride tablets to regulate sexual function if necessary.
3. Local inflammation
Long-term friction between the foreskin and glans may induce chronic balanoposthitis, which manifests as redness, swelling, erosion or white discharge. Recurrent attacks may result in scarring phimosis. It needs to be kept dry and clean, and drugs such as clotrimazole cream, triamcinolone acetonide econazole cream, and potassium permanganate topical tablets can be used to control symptoms during the attack period.
4. Cancer risk
Long-term stimulation of carcinogens in smegma may increase the risk of penile cancer, especially when combined with HPV infection. Cauliflower-like masses or ulcerative lesions may be seen clinically. After diagnosis, surgical resection is required, along with injection of cisplatin and other chemotherapy drugs. Early intervention has a better prognosis.
5. Phimosis incarceration
Forcibly turning up the foreskin may cause incarceration, which may manifest as congestion, swelling or even necrosis of the glans. Emergency manual reduction or dorsal incision is required, and circumcision is performed later to prevent recurrence. In the acute stage, you can take ibuprofen sustained-release capsules to relieve pain, and use magnesium sulfate solution as a wet compress to reduce swelling.
People with excessive foreskin should insist on washing with warm water every day and completely expose the glans, and avoid using irritating lotions. Choose breathable cotton underwear and clean it before and after sex. If you experience abnormal urination, increased secretions, or the foreskin cannot be repositioned, you need to seek medical treatment promptly. Those who have not undergone surgery are recommended to check for HPV and penile lesions every year. The diet should be light, limit the intake of spicy food, and appropriately supplement vitamin B to enhance mucosal resistance.
